A. Consent Notice

Before you continue, please review how Kwina will use your Personal Data. We will collect the information you provide, including your contact details and, where required for your selected plan, basic health, dietary, lifestyle or reproductive-health information. Kwina will use this information to create and deliver your selected plan, facilitate consultations, administer your subscription and provide support. Your consultation calls will be recorded for Kwina’s internal quality, training, compliance and service-improvement purposes. Your Personal Data will be protected and retained only as long as necessary or as required by law. You may withdraw consent and exercise your privacy rights using the links below. Please read the Privacy Policy before proceeding.

• I have read and understood the Privacy Policy and Consent Notice, and I consent to Kwina processing my Personal Data for the purposes stated above. (Required)
• I specifically consent to Kwina processing the health-related information I provide to prepare and deliver my selected plan. (Required)
• I consent to recording of my phone consultation(s) with Kwina consultants for internal quality, training, compliance and service-improvement purposes. (Required)
• I confirm I am 18 years or older and that the information I provide is true, accurate and complete. (Required)
• I consent to receiving promotional/marketing communications from Kwina. (Optional)

B. Detailed Consent Form

I confirm that I have been provided with a clear notice regarding the Personal Data requested by Kwina and the purposes for which it will be processed. I understand that I may withdraw consent at any time through the available mechanism, with effect for future processing based on consent.

• Health information consent: I consent to Kwina collecting and using the health, dietary, lifestyle and related information I provide to prepare and deliver my selected plan.
• Consultation recording consent: I consent to Kwina recording phone consultations with its consultants for internal quality, training, compliance and service-improvement purposes only. I understand that Kwina records every consultation call and that this consent is required to use the Services.
• Storage consent: I understand that Kwina will store my Personal Data for as long as necessary for the purposes stated in the Privacy Policy, subject to applicable legal retention requirements and Kwina’s data-purging procedures.
• The guidance is for wellness purposes and is not a substitute for professional medical advice. Kwina does not guarantee any particular health outcome, and ayurvedic and dietary guidance may work differently for different people.
• Fees paid for subscriptions and consultations are not refundable, except where a refund is required by law. Please see the Terms of Use.
• Please share and access your health details only through the Kwina platform.
• Rights: I understand that I may request access, correction, erasure and exercise other statutory rights, withdraw consent where applicable, and raise a grievance using the channels published by Kwina.
• Accuracy: I confirm that the information I provide is accurate and complete to the best of my knowledge and that I will update Kwina if material information changes.

Withdrawing consent and your rights

You can turn marketing messages on or off at any time on your profile. To withdraw any other consent, or to exercise your rights, email support@kwinawellness.com.
